Are you a food addict that's addicted to just one type of unhealthy food? Whether it's burgers, takeaway, crisps or chocolate, we want to hear from you.
Cheetah Television want to film food addicts being shown how to kick their addiction for a new Channel
Four series.

We'll send you an application form to fill in and email back along with a photo.
